There are four lookup methods: search by person, disease incident, foodborne illness (FBI) and by outbreak. The four search options appear as tabs.
When searching by person users can indicate multiple methods of search including name, social security number, date of birth, zip code, creation date, medical record number, or Soundex where users can search parameters based on phonics of the search name.
As with most screens, when a selection is made, the system automatically displays the closest possible match to patient name and the associated data.
Once the correct patient is selected, in this example the highlighted yellow line for "Abbey", the disease incident(s) connected to "Abbey" are listed below.
To select the incident the user clicks on the blue incident ID number.
