The community data entry form provides a tab for Patient and for Supplemental data. There is no Case Investigation tab since this function relates to Public Health activity.

The data entry form here allows reporting physicians to enter demographics that are transferred into the main WebvCMR system automatically.

This decreases data entry effort for health department users and improves the accuracy of the risk factor data.
